What We Do
Care management is a four part, ongoing process. It starts with a comprehensive in-home assessment and is followed by development of a care plan, implementation and monitoring of the plan as needed.
Some issues our care managers can assist with include:

Acting as a liaison to families at a distance, overseeing care, and quickly alerting families to problems – especially important when families are engaged in long distance caregiving for a loved one.

Acting as an advocate for family and clients in different situations - mom or dad finds themselves in the emergency room, at specialist medical appointments
, at care conferences -
Identifying problem areas at home, providing solutions and getting mom or dad to accept help or make changes

Screening, arranging, and monitoring in-home help or other services, including assistance in hiring a qualified caregiver for home.

Providing short- or long-term eldercare assistance for those engaged in local or long distance caregiving.

Providing guidance in crisis intervention or setting up a plan to prevent a crisis situation.

Assisting with facilitating and moving to or from a retirement complex, assisted care home, or nursing home.

Accompany clients to doctors or visit them in hospital - as health care professionals we know what questions to ask and will take away accurate information
